Opiate addict in long term recovery here 👋 I am in ongoing outpatient treatment still (I have 10 yrs clean in Sept ’20) and I want you folks to hear from someone in treatment, who can 100% confirm there is a HUGE uptick of new and returning opiate users and relapsed patients.i do recovery mentoring and let me tell ya, people are in distress. It’s not so much the stay home orders as it is the lack of leadership, the lack of alternatives to social norms (food bank options for singles or homeless, safe places to go during the day, places to earn a daily wage if not a regular job option). It’s all terrible. I really feel for my recovery folks right now.

    1. @Mulinaster That’s exactly the problem I mentioned: without travel restriction and contact tracing, opening can be troublesome. Virus doesn’t care where you live, but virus can’t spread where there is few people. Remoted rural area is usually fine b/c they don’t have population going in and virus spread very slowly in sparsely populated area.

    2. @Whenson Zhou Imagine this scenario: One person in a rural town carries it after the lockdown is lifted because there are “few people in rural areas so it’s fine!” That one person transmits it to a single other individual over the timespan of a month, no one detects it. Another month goes by, he transmits it to a single other individual, no one detects it. Eventually, EVERY state in the nation lifts travel restrictions, and that one person flies somewhere. The End, rural community just infected the entire nation because “rural areas have small populations.”

    3. @Mulinaster Well, it’s a possible scenario. But remember, despite the lock down, people are still getting infected constantly. So really, one infected person from rural area doesn’t change the picture at all. Think on a macro level: there will always be some peole getting infected in the society for such a contagious virus, but reopening isn’t impossible. A well prepared society can very effectively contained the virus when the number of infected people is low (like what the Koreans did, but US is neither well prepared nor at the low number of infected people). When the infected number is small enough, quickly identify and quarantine infected people is very good for both public health and economy. Sadly, this is not true for a lot of the states that reopen. Lifting lockdown & travel restriction without ways to contain is the way that your scenario will become true, and I fear that’s the case for US right now.

    4. @Whenson Zhou You’ve made my point for me: South Korea effectively limited the spread from getting into rural areas, but when local spread happens, it HAS To be dealt with effectively otherwise it presents an enormous problem for contact tracers. By closing everyone all at once and at least having a BASELINE target date for closure and re-opening, rather than “opening some states here, some places here due to ‘oh they’re rural small population!'”, you at least have the ability to make a goal and keep to it so that you know where every place should be at. The mitigated rollout scenario means that we will have a much more difficult time tracking down “who succeeded” in their quarantine and who didn’t, because at any given time, any spread vector could be because of either a slow rollout in one state or a quick rollout in another – because remember, it only takes one person who we didn’t track from a rural state to come into a populated state and we have NO idea where they came from. At least this way we can be relatively sure where these outbreaks are originating from. When a single person from a rural state can just march right in and infect everyone, the potential is so great for an undetected outbreak because we are all integrally interconnected throughout the states in some way. The only way to change this is to go full authoritarian like China, which isn’t going to happen. I just don’t see any logic behind the “it’s a rural place with a low population so it’s safer.” The epidemiological timeline holistically is what creates safety, not small numbers in single areas.

Interestingly, there are other countries in lockdown who are reporting fewer overall deaths than usual, because of things like cleaner air, fewer vehicle and work related accidents, less murders, less alcohol related deaths etc… Watching the different ways that different places are being affected is interesting.
