My heart just sank watching this. As a parent, I would be guilt ridden enough. No need to play the blame game. We don’t know the circumstances. (Although no matter the circumstance that shouldn’t have happened) we should stop focussing on what the parents have done wrong and instead focus on what can be done to prevent this in the future.
There is a safety key for that machine. The key should not have been in the machine and it could have also been unplugged when not in use.

I have a tread mill. It won’t run unless the magnetic key is attached. It’s a safety feature. Either the machine lacks this feature or the parent is negligent.
