To this day, drivers in Canada are not mandated fog lights in front and back…and instructed ( in driving schools for example ), how to drive in inclement weather…?

401 is known as the Highway of Death, It’s stupid how they designed it, one exit takes you opposite way, all of the signs specify the closest street rather than the nearest city, every ramp loops around and in some parts three lanes converge into one, I’m not surprised at all this would happen.
